By joining these martial arts classes, I give full consent and waive any legal rights related to potential injuries. This form serves as a release of liability for any damages that may occur during training. In the event of personal injury, I agree that no participating members (students, instructors, or owners of Master Jiu-Jitsu Academy/Grappling Zone LLC) will be held accountable, liable, or responsible for any said damages. By signing below, I acknowledge and agree to this waiver and release the Master Jiu-Jitsu Academy/Grappling Zone Inc., located at 3941 FM 521, Fresno, TX 77545, from any liability.
I understand that there is a 60-day cancellation policy in place, and I must sign and date a cancellation form in order to discontinue my membership.
